Immediate on the table result of #beforeandafter of a primary rhinoplasty. This wonderful, 22 year old woman, wanted to refine her trip. She had nasal breathing surgery at the same time, to correct her nasal septum and nasal valve repair.

His septum was collapsed in the front, replaced by scar, and deviated causing obstruction. He underwent an open structure septo-rhinoplasty for reconstruction. Very rewarding case of a revision rhinoplasty 6 month post operatively. This wonderful woman had a prior surgery that left her with a pollybeak deformity and nasal septal perforation.
Pollybeak deformity refers to excess tissue over the “supratip” area (region over the bridge of the nose right before the nasal tip). It’s name is derived from the beak-like shape of the nose. This usually occurs from prior rhinoplasty surgery, and can happen when dorsal humps are aggressively taken down without accounting for skin redraping. Patients subsequently develop excess scar tissue underneath the skin that gives the characteristic appearance. Other causes include inadequate dorsal hump reduction, as well as failed corrective surgery of a “tension” tip deformity.
She elected to undergo revision rhinoplasty with her own rib for aesthetic and functional purposes. The prior surgery also had left her with a large hole in her septum, for which she also underwent flap reconstruction for closure.
This wonderful patient underwent primary rhinoplasty after sustaining a severe trauma! She’s only 6 weeks out in these before and after rhinoplasty photos, but looks so great already!

Very excited to share photos of my patient. It’s hard to believe he is only 4 weeks out and looks so good! He had severe tip asymmetry, with inability to breathing, requiring overall reconstruction with rib. He had narrowing of his nostrils, in addition to being too projected (long nose). The key was to de-project him, but not by a lot, since he has a strong facial skeleton, so as to keep the overall harmony and balance of the face.
Excited to share before and after photos after reconstructive plastic surgery. This wonderful Park Avenue patient had three prior rhinoplasties that left him scarred and unable to breathe properly. This is what is called a “tertiary” rhinoplasty, and is very challenging from a surgical perspective. Each surgery he underwent increased the amount of scar in the nose, and reduced the available blood flow and vascularity. Adding to the difficulties is that his prior rhinoplasty surgeries were “reductive”, meaning that the original surgeons had removed critical portions of the nose, leaving the nose weakened and non-functional over time, causing significant collapse. I was fortunate to be entrusted with his fourth (and hopefully final!) revision rhinoplasty using his own rib to reconstruct the critical portions of his nose. He is 9 weeks postoperative in this photo, with some residual swelling, but is breathing again for the first time in years, and looking great!

It’s always a little extra pressure when you operate on someone you work with, but also an honor and a privilege to be entrusted with their care. She had an open rhinoplasty 8 months ago. She has mild tip swelling that should subside over the coming months, with de-rotation of her tip. Special thanks to her for allowing us to share in her journey!
